All new clients are expected to complete the Client Intake Form provided below in either PDF or MS Word format. PDF Form MS Word Form Loudoun Pediatric Speech Services now participates as an in-network provider for Blue Cross Blue Shield included its network; Anthem, and CareFirst. As a client you will be expected to provide your insurance identification prior to initiating treatment. In addition you are responsible for your co-payment at the time of visit. Loudoun Pediatric Speech Services will file your claim on your behalf. All clients with other insurance providers are still expected to pay in full at the time of visit. You will receive a treatment note and receipt which can be used to submit to your insurance for out-of-network reimbursement. Individual invoices are available upon request. Insurance companies and plans all vary in benefits regarding speech therapy so you are encouraged to contact your insurance provider prior to initiating treatment.
